Restless leg syndrome (RLS) causes an irresistible urge to move the legs, usually accompanied by uncomfortable crawling, tingling, or aching sensations that worsen at rest and in the evening. In TCM, RLS typically involves Liver Blood deficiency failing to nourish the sinews (muscles and tendons), combined with internal Wind stirring in the channels. The legs are traversed by multiple meridians, and when Blood is insufficient to anchor them, Wind movement creates the characteristic crawling sensations.
Treatment nourishes Liver Blood and extinguishes Wind using LR8 (Ququan, the Water point of the Liver channel that strongly nourishes Liver Blood), SP10 (Xuehai, "Sea of Blood"), and GB34 (Yanglingquan, the Influential point for sinews). SP6 (Sanyinjiao) tonifies Blood across the three Yin channels of the leg. Local points ST36 (Zusanli) and GB39 (Xuanzhong) calm the affected lower limbs. LR3 (Taichong) subdues Liver Wind.
A 2012 study in Sleep Medicine found acupuncture improved RLS severity scores and sleep quality. Treatment is particularly effective when iron deficiency — a common RLS contributor — is simultaneously addressed. Sessions scheduled in the afternoon or early evening, before symptoms typically peak, produce the best results. Eight to twelve treatments are usually needed for sustained improvement.
In TCM, night is Yin time when Blood should settle inward. If Liver Blood is deficient, the sinews of the legs are inadequately nourished as Blood recedes, and internal Wind fills the vacuum, creating the crawling sensations and urge to move. Western medicine links it to circadian dopamine fluctuations.
Research shows acupuncture can improve RLS severity and sleep quality. By nourishing Liver Blood and calming internal Wind, it addresses the root pattern. Results typically build over eight to twelve sessions, with the most noticeable improvement in sleep quality and the urgency of leg sensations.
