Achilles Tendinopathy and Acupuncture

Achilles tendinopathy involves degeneration and pain in the Achilles tendon, the largest tendon in the body. It results from repetitive overloading, causing failed healing and disorganised collagen fibres. Risk factors include sudden increases in running volume, tight calf muscles, flat feet, and fluoroquinolone antibiotic use.

In TCM, the Achilles tendon lies on the Bladder and Kidney channels, and tendinopathy reflects kidney qi deficiency combined with local blood stasis. Treatment uses BL-60 (Kunlun) beside the tendon, KI-3 (Taixi) on the medial ankle, and BL-57 (Chengshan) in the calf to relax the gastrocnemius-soleus complex. Electroacupuncture along the tendon at low frequency (2 Hz) stimulates fibroblast activity and tendon remodelling.

A 2013 randomised trial in Knee Surgery, Sports Traumatology, Arthroscopy found that acupuncture combined with eccentric exercises produced greater improvements in pain and function at 12 weeks than eccentric exercises alone, with benefits maintained at one-year follow-up.
