Thoracic Outlet Syndrome and Acupuncture

Thoracic outlet syndrome (TOS) occurs when nerves or blood vessels between the collarbone and first rib become compressed, causing pain, numbness, and tingling in the shoulder, arm, and fingers. It is commonly associated with repetitive overhead movements, poor posture, or anatomical variations such as a cervical rib.

In TCM, TOS relates to qi and blood stagnation in the channels of the neck and upper limb, often compounded by muscular tension obstructing flow through the Lung, Heart, and Small Intestine meridians. Acupuncture treatment targets points including SI-3 (Houxi), LU-1 (Zhongfu) to open the chest, GB-21 (Jianjing) to release the trapezius and scalene muscles, and LI-4 (Hegu) to restore circulation to the hand. Trigger point needling of the anterior and middle scalene muscles can directly decompress the neurovascular bundle.

A 2019 study in Evidence-Based Complementary and Alternative Medicine reported that acupuncture combined with physical therapy improved nerve conduction velocity and reduced pain scores in TOS patients more effectively than physical therapy alone.
