Injury creates local Blood stasis and disrupts channel flow, while underlying Qi and Blood deficiency impairs the tendon's self-repair capacity.
Point selection targets each affected muscle's channel. Supraspinatus (most commonly injured): SJ14 (Jianliao) and LI16 (Jugu). Infraspinatus: SI10 (Naoshu) and SI11 (Tianzong). Subscapularis: LU1 (Zhongfu) and LI15 (Jianyu). Ashi points at the specific site of tenderness are essential. GB21 (Jianjing) releases the upper trapezius, which often compensates for rotator cuff weakness. Distal points LI4 (Hegu) and SJ5 (Waiguan) promote upper limb circulation.
Electroacupuncture across the injured tendon promotes healing by increasing collagen synthesis and local blood flow. For partial tears, acupuncture combined with progressive rehabilitation exercises can avoid surgery in many cases. Full-thickness tears may still require surgical repair, but acupuncture supports pre- and post-operative recovery. Eight to fifteen sessions are typical depending on injury severity.
