Perimenopause and Acupuncture

Perimenopause is the transitional phase leading to menopause, typically beginning in a woman's early to mid-40s and lasting four to eight years. During this time, oestrogen and progesterone levels fluctuate unpredictably, causing irregular cycles, hot flushes, night sweats, mood changes, and sleep disturbances.

TCM views perimenopause as the natural decline of kidney yin and the Tian Gui (reproductive essence), with symptoms arising from the resulting yin-yang imbalance. When kidney yin becomes deficient, empty heat rises — manifesting as hot flushes, night sweats, and irritability. Treatment nourishes kidney yin using KI-6 (Zhaohai), KI-2 (Rangu) to clear deficiency heat, and HT-6 (Yinxi) to calm night sweats. SP-6 (Sanyinjiao) regulates the three yin meridians and is central to gynaecological treatment.

The landmark ACUFLASH trial, published in Menopause (2009), demonstrated that acupuncture reduced hot flush frequency by 50% and improved sleep quality, with effects persisting three months after treatment concluded.

Frequently Asked Questions

At what age does perimenopause typically start?

Perimenopause most commonly begins between ages 42 and 47, though it can start in the late 30s. Early signs include changes in menstrual cycle length, heavier or lighter periods, and the onset of hot flushes.

How does acupuncture help with perimenopause symptoms?

Acupuncture helps regulate the hormonal fluctuations of perimenopause by modulating the hypothalamic-pituitary-ovarian axis. It can reduce hot flush frequency, improve sleep quality, stabilise mood, and ease the overall transition.
