Golfer's elbow, or medial epicondylitis, involves pain and tenderness at the inner elbow where the forearm flexor tendons attach to the medial epicondyle. Despite its name, it is common in anyone performing repetitive gripping, wrist flexion, or forearm pronation, including office workers, climbers, and tradespeople.
TCM views medial epicondylitis as qi and blood stagnation in the Heart and Pericardium channels, which traverse the medial elbow. Treatment focuses on restoring circulation to the tendon insertion. Key acupuncture points include HT-3 (Shaohai) at the medial elbow crease, PC-3 (Quze) for the pericardium channel, and local ashi points directly over the tender epicondyle. Electroacupuncture at 2-4 Hz applied across the affected tendon has been shown to increase local blood flow and stimulate collagen repair.
Research in the British Journal of Sports Medicine has demonstrated that acupuncture produces superior pain relief compared to corticosteroid injection at six-month follow-up, with corticosteroid groups showing higher recurrence rates.
Tennis elbow (lateral epicondylitis) affects the outside of the elbow where the wrist extensor tendons attach, while golfer's elbow (medial epicondylitis) affects the inside where the flexor tendons attach. Both involve tendon overuse but affect different muscle groups and different sides of the elbow.
Clinical evidence supports acupuncture for medial epicondylitis, particularly when combined with eccentric exercises. Acupuncture helps by increasing blood flow to the poorly vascularised tendon insertion, reducing pain signals, and promoting tissue repair. Many patients experience significant improvement within four to eight treatments.
