The condition involves central pain sensitisation — the nervous system amplifies pain signals. In TCM, fibromyalgia reflects Qi and Blood deficiency with concurrent Liver Qi stagnation. The deficiency creates generalised aching and fatigue, while stagnation produces the tender points and pain that wanders throughout the body. Dampness often contributes to the heaviness and fog.
Treatment uses a gentle approach — fibromyalgia patients are often sensitive to strong stimulation. ST36 (Zusanli) and SP6 (Sanyinjiao) tonify Qi and Blood systemically. LR3 (Taichong) moves stagnant Liver Qi. GV20 (Baihui) addresses fatigue and brain fog. Ashi points at the most active tender points are needled lightly. SP9 (Yinlingquan) resolves Dampness, and HT7 (Shenmen) supports sleep quality. Auricular acupuncture at Shenmen and Zero Point calms the sensitised nervous system.
A 2019 Cochrane review found acupuncture improved pain and quality of life in fibromyalgia, with effects lasting beyond the treatment period. Electroacupuncture at low frequency (2Hz) stimulates endorphin and enkephalin release, which helps recalibrate the pain processing system. Treatment should begin conservatively — minimal needles, short retention — and gradually increase as tolerance develops. Twelve to twenty sessions are typical.
