Moxibustion for Breech Baby Turning

Moxibustion applied to BL67 (Zhiyin) at the outer corner of the little toenail is a well-researched technique for encouraging breech babies to turn to a head-down position. Treatment is typically performed between 33-36 weeks gestation, with the mother in a reclining position. A moxa stick is held close to BL67 bilaterally for 15-20 minutes daily for up to 10 consecutive days. The technique is believed to increase foetal activity, stimulating the baby to somersault into the optimal cephalic (head-down) presentation.

A landmark Italian randomised controlled trial published in JAMA (1998) found that moxibustion at BL67 resulted in a 75% cephalic presentation rate compared to 48% in the control group. The mechanism is thought to involve stimulation of the adrenal cortex, increasing oestrogen and prostaglandin levels, which in turn increases uterine contractility and foetal movement. The treatment is most effective between 33-35 weeks when there is still adequate amniotic fluid and space for the baby to turn.
