Cupping therapy uses suction cups placed on the skin to create negative pressure, drawing blood flow to the surface and releasing muscle tension. Traditional fire cupping uses glass cups heated with a flame to create a vacuum, while modern silicone or pump cups achieve suction mechanically. Cupping is commonly applied along the Bladder meridian on the back, the Gallbladder meridian on the shoulders, and over areas of muscular tension and pain. The circular marks left by cupping are not bruises but petechiae caused by capillary dilation, typically fading within 3-10 days.
TCM uses cupping for several purposes: dispelling Cold and Dampness from the channels (back pain and stiffness worsened by weather changes), releasing the exterior during Wind-Cold invasion (early colds and flu), invigorating Blood flow to relieve stagnation (chronic pain), and drawing out pathogenic factors. Sliding cupping — moving an oiled cup across the skin — provides the additional benefit of fascial release. Flash cupping — repeatedly applying and quickly removing cups — is gentler and used for more deficient or sensitive patients.
Those may recognise cupping from its use by athletes for muscle recovery. In clinical practice, it is combined with acupuncture to address both superficial muscular issues and deeper constitutional patterns.
The colour and darkness of cupping marks provide diagnostic information. Darker purple marks indicate greater Blood Stasis in the area. Pale pink marks suggest less stagnation. Marks that appear quickly and are very dark may indicate chronic muscular tension or poor circulation. As treatment progresses and circulation improves, marks typically become lighter and fade faster.
For acute conditions like early colds or muscle spasm, cupping may be done every 2-3 days. For chronic conditions, weekly sessions are typical. Cups should not be placed on the same location until previous marks have fully faded. A standard cupping session retains cups for 5-15 minutes depending on the individual's constitution and the condition being treated.
