Shoulder Impingement Syndrome and Acupuncture

Shoulder impingement syndrome occurs when the rotator cuff tendons, particularly the supraspinatus, become compressed beneath the acromion during overhead arm movements. This causes pain with reaching, lifting, and sleeping on the affected side. The condition is common among swimmers, painters, and anyone performing repetitive overhead activities.

In TCM, shoulder impingement falls under shoulder Bi syndrome involving stagnation of qi and blood in the local channels, particularly the Large Intestine and Triple Energiser meridians. Key acupuncture points include LI-15 (Jianyu) at the anterolateral shoulder, TE-14 (Jianliao) at the posterolateral aspect, and SI-9 (Jianzhen) posteriorly. Distal points such as ST-38 (Tiaokou), a classical empirical point for shoulder mobility, are frequently needled contralaterally while the patient moves the affected arm.
